Metatarsalgia (Ball of Foot Pain)

Metatarsalgia is a term for pain and inflammation under the ball of the foot. Many different conditions can be grouped under it. At Sports and Structural Podiatry in Maroochydore, we work out what is causing your forefoot pain and build a plan with you to settle it.

What it is

Common causes of ball of foot pain

Pain under the ball of the foot can come from several structures. Common causes include:

Capsulitis

Inflammation of the joint capsule, the envelope around a joint. It often affects the joint under the second toe and is commonly seen alongside a bunion.

Synovitis

Inflammation of the lining of a joint, which can cause swelling, warmth and pain when the joint moves.

Bursitis

Inflammation of a bursa, one of the small fluid-filled sacs that cushion the joints.

Other causes

Nerve irritation, bone stress injuries and arthritis can also cause forefoot pain. Imaging such as ultrasound or X-ray is sometimes needed to tell them apart.

Your first visit

What happens at your assessment

Your first appointment usually covers four things.

01

Your history

When the pain started, what makes it better or worse, your work and activity levels, your footwear and any previous treatment.

02

Examination

We examine the area to find where the pain is coming from and check for other conditions that can cause similar symptoms.

03

Movement and footwear

We look at how your feet and legs move and load when you walk, and at the shoes you wear most days.

04

A clear plan

We explain what we found in plain language and agree a treatment plan with you, including what you can keep doing while it settles.

Treatment

Treatment options

The main goal is to take load off the sore area and spread it across the whole foot. Depending on the cause, options may include:

Offloading

Simple padding in the shoe or toe spacers to spread pressure across the forefoot.

Footwear advice

Guidance on shoes with enough room and cushioning for the forefoot.

Custom orthotics

Made from a 3D scan of your feet and an individual prescription, where your assessment shows they are appropriate for you.

Foot mobilisation therapy (FMT)

A hands-on therapy where gentle, precise pressure is used to move the joints of the foot and reduce joint restriction.

Strength and stretching programs

Exercises for the foot and calf, progressed as your foot settles.

MLS laser therapy

A non-invasive treatment using light energy, used alongside other care where it is suitable for you. Results vary from person to person.

We usually start with simple treatments and only move to more involved options when they are needed. If imaging or a surgical opinion is appropriate, we will explain why and arrange a referral.

Your podiatrist will talk you through the options, expected benefits and any risks before you decide.

When to book

Book in if pain under the ball of your foot lasts more than a week or two, or is changing the way you walk.

If the forefoot is suddenly very swollen, hot and red, or you cannot put weight on it, seek a medical assessment promptly.
